Anonymous Hazing Reporting Form
This form is used to report an alleged crime and/or safety concern to Northwest Nazarene University. This form is NOT A POLICE REPORT, but it may generate a follow-up by N麻豆原创 Campus Safety or other agencies for further information gathering and investigation. If you are intending to file a police report, please call the Nampa Police Department at 208.465.2257 or your local law enforcement agency to do so. If you have received communication in the past about your status as a Campus Security Authority related to the Clery Act, you are a mandatory reporter and not permitted to report a crime anonymously. If you are not a Campus Security Authority, you may use the anonymous reporting feature of this form.
